The Care Manager plays a vital role within our interdisciplinary
team, focusing on assessing, planning, advocating, and coordinating
patient care from the moment of admission through to discharge.
This position ensures a smooth transition following
hospitalization, aligning with our organization's mission, values,
and service standards. Our Care Management model emphasizes
effective transition planning and oversight of length of stay,
aiming to enhance patient experience, safety, and quality of care
by utilizing performance metrics and implementing best practices.
Position Overview: Educational Requirements: • Graduate from an
accredited Nursing School is essential. • A Bachelor’s Degree in
Nursing is a minimum requirement. Experience: • Preferred 2-3 years
of experience in an acute care hospital environment. • Experience
in Care Management or Utilization Management is advantageous.
Licensure: • A current nursing license in the state of employment
is mandatory. Certifications: • Basic Life Support (BLS)
certification is required at the time of hire or within 90 days of
starting. • Certification in Case Management is preferred. Why Join
