Job Title: Case Manager RN - Home Health Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum of 2 years of home health experience as a case manager License: Registered Nurse License Certifications: BLS, OASIS documentation experience Must-Have: - Home health experience as a case manager - Local candidates are highly preferred - Recent experience in home health, not hospice Description: This position is responsible for the overall management of patients, including the provision of direct care in accordance with current standards and regulations. The role plans, organizes, and directs all patient care services for patients in the assigned caseload. It ensures the optimal degree of quality care is maintained. The responsibilities include coordinating the interdisciplinary team to implement the established plan of patient care. The position is also responsible for managing visit utilization and controlling expenses. Delivers holistic and individualized care to all patients in the assigned area. Develops, implements, and manages an interdisciplinary plan of care that considers psychological, sociocultural, spiritual, economic, and lifestyle factors. Fosters and maintains collaborative relationships between patients, their families, physicians, and other healthcare providers through effective communication.
