The RN Care Manager is responsible for overseeing and coordinating
the transitions of patient care from the point of admission through
to discharge. This role emphasizes maintaining safety and quality
during hospitalization and after discharge. Collaborating with an
interdisciplinary team, the Care Manager advocates for patients
while employing best practices and performance metrics to enhance
both the duration of stay and overall patient experience. Discover

team, the Care Manager evaluates, plans, advocates, and coordinates
care throughout the patient’s journey, ensuring a smooth transition
following hospitalization. Responsibilities are carried out in
alignment with our organization's mission, values, and service
standards. The Care Management model focuses on effective
transition planning and monitoring length of stay to uphold patient
experience, safety, and quality of care, utilizing performance
metrics and best practice adoption. Position Details: Required
Education: • Graduate from an accredited Nursing School • Bachelor
of Science in Nursing Experience: • 2-3 years in an acute care
hospital environment, preferred • Experience in Care Management or
Utilization Management, preferred Licensure: • Active nursing
license in the state of employment, required Certifications: • BLS
(CPR) certification required at the time of hire or within 90 days
of employment • Certification in Case Management is preferred Why
